Generally, the medical insurance is divided into hospital benefits, and hospital and surgical policies. Insurance may comprehensively cover hospital expenses such as operations, scans and x-rays, which is usually paid in the form of a pre set amount for each day of hospitalization. Certain health insurance policies cover expenses for doctor visits, hospitalization, prescription drugs and other health-care costs.

You would be issued a document know as policy in exchange for the premiums. The policy includes a list of health care providers that you can visit. If you choose a provider not mentioned on the list, the insurance company may not pay the full amount for the services.

The federal and state laws that govern insurance companies vary from one state to another. If you travel, ensure that your coverage also follows you.

The insurance policy states the mode of payment for your medical bills. For example, it may pay for regular visits to a doctor, and for this you need your yearly physical exam. This would involve filling out insurance forms at the doctor's office. If your insurance includes payment for prescription medicine, then you would receive the prescribed medicines at no charge.
